What is armodafinil, and how does it work?

Armodafinil is a wakefulness-promoting medicine. It's the longer-lasting half of modafinil, the drug many Australians know by the local brand name Modavigil.

Here's the chemistry in plain English. Modafinil is a mix of two mirror-image molecules, called the R and S forms. Armodafinil is only the R form. That R form hangs around in the body longer, which is why armodafinil tends to feel steadier through the afternoon rather than tapering off after lunch. It isn't a classic stimulant like caffeine or dexamphetamine. Researchers believe it works mainly by blocking the reuptake of dopamine, with knock-on effects on orexin, histamine and noradrenaline, the brain's own "stay awake" signals. The result, for the right patient, is alertness without the jittery rush.

What it's approved for in Australia

The TGA assessed armodafinil as a new chemical entity and approved it as a prescription medicine in 2016. Its approved uses mirror modafinil's:

• Narcolepsy, to reduce excessive daytime sleepiness
• Obstructive sleep apnoea (OSA), for leftover sleepiness despite proper CPAP treatment
• Shift work disorder, for people whose rosters wreck their natural sleep cycle

Using it to cram for exams or push through a deadline is "off-label", and the TGA has publicly warned against this so-called smart drug use.

Armodafinil usually reaches peak levels around two hours after a dose. Its half-life is roughly 15 hours, so a morning tablet can still be active at bedtime. That's the single most common reason people report poor sleep on it: they took it too late in the day.

Both Artvigil and Waklert are Indian generic versions of armodafinil. They were made for the Indian domestic market and later became popular online because they cost a fraction of the original US brand, Nuvigil.

Artvigil 150mg

Artvigil is made by HAB Pharmaceuticals & Research, a mid-sized manufacturer based in India. Each tablet contains 150mg of armodafinil and typically comes in blister strips of 10. It's generally the cheaper of the two online, which goes a long way towards explaining its popularity with students and gig workers overseas. Users often describe Artvigil as a smooth, even lift. Some say it feels slightly milder than Waklert, though there's no published head-to-head study to back that up.

Waklert 150mg

Waklert is made by Sun Pharmaceutical Industries, one of the largest generic drug makers in the world. Sun Pharma has a long track record supplying regulated markets, including the US, which gives some buyers extra confidence in its quality systems. 

Waklert also comes in 150mg tablets, usually in strips of 10. Anecdotally, people describe it as a touch sharper or more "focused" than Artvigil. Again, that's user opinion, not clinical evidence.

Why two tablets with the same dose can feel different

If the active ingredient and dose are identical, why do people swear one feels different? A few honest reasons:

• Inactive ingredients :  Fillers, binders and coatings affect how fast a tablet breaks down in your stomach.
• Manufacturing tolerance. : Legitimate generics must fall within a set range of the labelled dose, so small batch-to-batch differences exist.
• Expectation. If a forum tells you Waklert is stronger, your brain may well agree.
• Counterfeits. Popular brand names are often copied, and a fake tablet can contain anything, or nothing.

2. Side effect profile : Because the drug is the same, the side effects are the same: headache, dry mouth, nausea and trouble sleeping lead the list. A handful of users report fewer headaches on one brand over the other, but it's usually a personal quirk rather than a pattern.

• Some antidepressants, anticoagulants and anti-epileptics : Armodafinil affects liver enzymes that break these down.
• Alcohol : It won't cancel out intoxication, and mixing the two can mask how impaired you really are.
• Caffeine : Stacking strong coffee on top often makes jitters and headaches worse.

Who should be especially careful

People with heart conditions, high blood pressure, a history of mental illness or liver problems need a careful assessment first. Armodafinil isn't recommended during pregnancy or breastfeeding. These are exactly the conversations an online seller will never have with you.
